COOKING CLASS REGISTRATION FORM

Please fill out the registration form (below) if this is your first session with us.
Please note a complete registration form will be required to partcipate. Why? Because the form covers our terms and conditions for participating and helps to provide us with data in line with our GDPR Policy that enables us to secure ongoing funding as a Community Benefit Society and build financial resilience.
I have previously attended a class, do I need to fill out another registration form?
No. We will have your details on the database.
PLEASE NOTE: If you are buying more than one ticket, your guests also need to fill out registration forms. They can do so here; you can send them the link to this page. This helps to save time but they (and you) are also more than welcome to do this in class. We have paper copies.
I would prefer to fill the registration form in class, can I do this?
Yes (see above).

REGISTRATION FORM:
As a provider of services directly to the public and for private, public and voluntary sector organisations it is necessary for Lifeafterhummus Community Benefit Society to collect and process personal data. As our services generate health and wellbeing outcomes for individuals, it is also necessary for us to process sensitive personal data which helps us demonstrate the impact of our services. One of the ways we achieve this is by collecting data from this registration form. At the end of the session you will be asked to fill out an evaluation form. This is a legitimate interest as it is both crucial to our commercial viability (demonstrating capability and impact including to our funders and our curriculum provider) and the achievement of our objects as a Community Benefit Society, a copy of which you can see on our website: www.lifeafterhummus.com After the class you will find the majority of resources posted on our facebook page: www.facebook.com/lifeafterhummus however we may also need to email resources to you for which we will use your email address below. FAQs -FREQUENTLY ASKED QUESTIONS:

How much is the ticket:
Tickets are priced accessibly at £14.02 including PayPal fees.
We also offer 2x concessionary priced tickets at £6 each. These are for people who would not normally be able to afford full priced tickets whether you are working/ student/ unemployed – if this is you, please drop us an email (details below). If tickets are no longer available for this session you can ask to be put on to a list for future sessions, if you wish.
We always need volunteers, if you have attended our sessions before and would like to volunteer, do ask us in person/ email us (details below).
Where is my ticket confirmation?
Your PayPal receipt is your ticket confirmation. Please check your emails.
What is the address again?
Somers Town Community Assocition, 150 Ossulston Street, London NW1 1EE – We are less than 5 minutes walk from St Pancras internation station.
What time does the class start?
If you have booked for Friday, the doors open at 6.15pm and the session starts at 6.40pm. With a view to ending at 9/ 9.30pm.
If you have booked for Saturday, the doors open at 3.45pm and the session starts at 4.10pm. With a view to ending at 6.30/ 7pm.
*Please bring containers for leftover foods.
Refunds:
We offer refunds 18 days prior to the session. Please email us joana[at]lifeafterhummus.com to request this. This must be done by email (as it is easy to miss DMs and comments on social media).
If a guest chef unexpectedly has to cancel their guest spot within 18 days in the run-up to the event (these things can happen), we reserve the right to proceed with the class with a suitable replacement. The event will proceed and tickets will not be refunded.
Resale of tickets/ Purchasing tickets for a guest:
You are welcome to purchase tickets on behalf of others; please do let them know that they will have to fill out registration forms prior to the event. You can resell your ticket but this is between you and the buyer. The buyer must present with the name of the original ticket holder and the email address used to purchase the ticket. The new owner of the ticket will have to also fill out a registration form. If you are buying a ticket from a person this is between you and them; do be aware of scams/fake accounts. Ask for example; for a copy of the paypal email confirmation. You are more than welcome to check with us if we have that name on our database. But bear with us for a reply; we are very busy in the office. We will not be held responsible for fake tickets sold. Sadly there are people out there trying to scam people out of money even for cooking classes!
Allergens:
Food prepared during this class may contain COMMON ALLERGENS, and we do not assume any risk(s) of personal injury or illness that may result from consuming or handling this food. Thou we cannot guarantee there has been no cross contamination, what we do suggest to those who suffer from allergies is to watch the healthy techniques being used and to replicate this at home without the allergens.
Family/ Child friendly:
This is a family friendly event and tickets must be purchased for children over the age of 6 years old. Under 6 years old are welcome to attend for free. A ticket does not need to be purchased for them. However, we do request 1 adult per 2 under 6 years old. We will ask you to sign consent as guardian for any under 16 year olds. If you are not their legal guardian we require a letter from their legal guardian confirming you are their chaperone.
Please note: We do also provide no-cut gloves for the little ones. This helps to build up confidence in the kitchen.
Photography:
We ask you to give consent on your registration form, with respect to any film, photography, video or audio recordings being made with respect to your participation in this class. This is mandatory. Why? As a community benefit society we need to be able to demonstrate the work we do in order to build financial resilience/ sustainability. You are most welcome to also take pictures, but please do check with those in your group that they are happy to be pictured. Guest chefs are also able to take their own photography/ film.
